Cum pot sa atasez pe FORUM un chestionar ceva mai complicat

Cum pot sa generez un chestionar care sa permita si adaugarea unor informatii direct de catre cel care completeaza chestionarul?
De exemplu:
Nume si prenum:......
Sex: M/F
Varsta: .....
Dmeniul de interese: [assaa/asdad/dsfwas/sfdsa...] lista
etc...
